Stephanie Craft, MSW, LSW is available for consultation to all of our patients and families. Her services are free of charge. Stephanie documents the level of stress that patients and/or families may be under and assists them with the problems or difficulties that he/she may be facing.
Services provided:
- Assist with financial/insurance questions and concerns
- Transportation assistance
- Medication assistance
- Homecare and hospice assistance
- DME (durable medical equipment)
- Assisting patients in establishing primary care physicians
- Nursing home placement/Short term rehabilitation
- Distress management/psychosocial evaluations
