Medical Records

To access your records of care at Southern Ohio Medical Center or SOMC clinics, you can:

• Log into the patient portal if you’ve set up an account, which you can do after making your first appointment with us
• Call (740) 356-8786 on weekdays 8 a.m. to 4 p.m. to reach our Health Information Management office

Health Information Management can:

• Provide copies of your records on paper or a compact disc (CD)
• Release certain records you request to a provider outside of SOMC

You or your provider can call us or send a fax to 740-356-6326 to request your records.

SOMC providers already have access to records of your care with us, so you don’t need to request a medical records release for SOMC appointments.
